NDIS Plan Reassessments Have Changed. What You Need to Know

The NDIS has changed the way participants can ask for an unscheduled plan reassessment. If your support needs have changed and your current plan is no longer meeting those needs, it is important to understand the new process.

What is a plan reassessment?

A plan reassessment is when the NDIS looks at whether you need a new plan because your support needs have changed significantly.

You may need a reassessment if your:
- disability support needs have increased.
- living situation has changed.
- informal supports have changed
- current plan is no longer meeting your needs.

These changes need to be significant and ongoing, if they are temporary you wont be able to get for a reassessment. 

Who can ask for a plan reassessment?

Under the new rules, only the following people can request a plan reassessment:

- The participant
- A plan nominee
- A child representative

Support coordinators, providers, therapists and family members can still help gather information and evidence, but they cannot submit the request unless they are thplan reassessment?e participant's authorised nominee or child representative.

What are the new requirements?

To request a plan reassessment, you must:

- Complete the NDIS Plan Reassessment Request Form.
- Explain what has changed.
- Explain why your current plan is no longer meeting       your needs.
- Provide evidence to support the request.

What information should you collect?

The evidence should clearly show:

  • What has changed since your current plan was approved.
  • How the change affects your daily life.
  • Why your current supports are no longer enough.
  • Why you need different or additional supports.

Evidence can include:

  • Reports from your doctor
  • Therapy reports
  • Functional assessments
  • Information from support workers and other professionals involved in your supports

The NDIS generally wants recent evidence, so make sure your reports and assessments are up to date.

How do you request a reassessment?

Once you have completed the form and collected your evidence, you can submit it to the NDIS.

The form and supporting evidence can then be submitted through the NDIS Service Hub, by mail, or through your local NDIS office or My NDIS Contact.

What about plan variations?

Plan variations are still available.

A plan variation may be a better option when a change is:

  • Urgent
  • Minor
  • Short term

For example, if you need extra support while recovering from surgery or due to a temporary change in your circumstances, a plan variation may be more appropriate than a full plan reassessment.
